How to create a survey on WordPress

Now we will explain how to create a survey for your WordPress website. To follow along, you will need Gravity Forms installed or enabled on your site. Alternatively, you can sign up for a free Gravity Forms demo, where you can access all of the plugin’s features and add-ons, including Survey!

Install the survey add-on

To get started, you first need to install the Survey add-on. To do this, in your WordPress dashboard, select Forms > Add-ons in the left menu. Then scroll down the add-on page until you see Investigation and click Install > Activate.

Create a new survey form/Select survey form template

Then click Forms > Add a new form to open a new form. Here you will have the option to use a survey template or create a survey from scratch.

For any new form, you will need to give it a name and a description (optional). Likewise, once installed, the Survey add-on can be used with any pre-existing form.

If you choose to use the prebuilt survey form, you will be able to fully customize this template in the form editor.

Here you can easily add or remove fields, change the survey field type (more information below), update the text to make it relevant to your survey requirements, configure survey settings every field and much more.

If you create a form from scratch, the Investigation the field can be located in the bank of Advance fields on the right.

To edit the Survey field, first insert your question in the blank space below Survey question. Then under Line of Inquiry TypeSelect the type of survey question you want to use.

THE Choice The Inquiry Box section is where you enter selections for any Line of Inquiry Type you choose. Use the plus and minus icons on the right to add or remove options.

Like most Gravity Forms fields, you can also enable conditional logic and dynamically populated data.

For each new survey question, you will need to add a new Investigation field – once you are finished, click Update to save your form. For more details on creating a survey, see the documentation – Survey Guide.

Configure form settings

Before you add your survey to a page and make it available to participants, you’ll want to configure a few settings that apply to the entire form. You will find them in Settings at the top of the form builder.

Below ConfirmationYou can change the message on the page that users see after submitting the survey. You’ll want to change the default confirmation to something like “Thanks for completing the survey!” We will respond to you shortly. »

Below Notificationsyou can configure the notification email(s) sent when someone submits a form.

The default admin notification will send a copy of responses to the WordPress admin. We also recommend creating a notification that emails survey participants with their responses or to send them their incentive.

Add your survey to a page or post

The final step is to embed your survey into a page or publish it on your site. It’s a quick task. It’s like integrating any other gravitational form.

You can use the Gravity Forms block in the Gutenberg editor (recommended) or add the shortcode in the classic editor. Read our guide on adding a form to your site to help you.

Review your results

Once your participants have had time to complete the survey, view the results by clicking the button Survey Results at the top of the form editor. Gravity Forms breaks down the results so you can get a clear overview of how your users answered each question.

If you want to view a person’s response separately, you’ll find it under Starters (next to Settings) in the form builder.
